Jan 12, 2024 · The iliopsoas muscle is a large compound pelvic muscle composed of the psoas major and iliacus. The iliopsoas belongs to the anterior compartment of the pelvic muscles and is a muscle of the posterior abdominal wall. It found within the iliopsoas compartment and is an important muscle in locomotion and upright posture.

Iliopsoas bursitis is characterized by distension of the iliopsoas muscle bursa due to synovial fluid and/or hypertrophic synovium. Hip disease is usually associated with it, however, isolated bursitis is also present.

The imaging characteristics of iliopsoas bursitis are a well-defined, thin-walled cystic mass with a communication to the hip joint and peripheral contrast enhancement. The most accurate way to assess iliopsoas bursitis is with MR imaging; thus, it should be used for accurate therapy planning and follow-up studies.

In cases of non-operative treatment failure, axial acetabular cup overhang measurement by CT-scan can be a useful tool in deciding between iliopsoas tenotomy or cup revision surgery in selected cases of very severe acetabular malposition.
